The concept of prevention is not practiced in so many aspects of our day to day life for which a real culture change is needed. For example, viewing the most recent tragic mass shooting in Parkland High School is a continuation of similar tragic events that have taken place. Since Dec 2012 when Sandy Hook Elementary School shooting killing 20 children, there has been 1607 mass shootings killing at least 1864 and wounding 6459 fellow Americans.

The increasing rate of Automation constitute a major concern for many as Artificial Intelligence (AI) has taken away first many routine low skilled jobs from middle class in America. This has gone in parallel with with increased income-inequalities between owners of capitol and those who do the labor. The share of income held by top 1% of the population increased to 20% up from 10% in 1980. AI and Automation influence has increased from affecting low skilled jobs, to having medium skilled jobs as Driver-less Cars, Driver-less buses to be operating in smart cities, leading to many drivers potentially loose their jobs. It is estimated that one fourth to one third of total jobs will be lost due to AI/Automation.

As the year 2016 comes to an end and 2017 is here, one must pause and rethink the New Year Resolution. To have a fresh start with the New Year and work on achieving a number of goals which usually include start exercising regularly, loose weight, stop smoking and drinking, spend more quality time with family to enjoy life more, help others, get organized and out of debt.

Aiming to strike a balance between Physical, Mental, Social and Spiritual needs of our body and soul, avoid stress and find activities to enjoy life which are not necessarily extravagant or self serving. Yes, your Heart Health will be optimized with improving your Physical Health, but also when you get out of your comfort zone to serve others without waiting for some kind of a reward. It has been clearly shown that random acts of kindness is a good thing for your Heart Health and also a way to thank our Creator for the endless blessings we took for granted. It is also an opportunity to start having daily self-assessment to be a better human being and to think about ways of improving your performance on the following day when you wake up to help those who are less fortunate.

In other words, to start the New year with spreading peace starting with yourself, then family, friends, coworkers, entire humanity and All Mighty God. This concept of comprehensive Wellness has been an important part of our Health and Wellness Program to go on for the seventh year in 2017 to be implemented not only for few hours or days but rather become a 24/7 life style Resolution. Have a Happy and Healthy New Year everybody!

Avoid-Holiday-Heart-Syndrome-1200x800.jpg
15/Dec/2018

Holiday Heart syndrome most commonly refers to association between Alcohol use and heart rhythm disturbances with Atrial fibrillation is most common. Alcohol increases the secretion of Epinephrine and Nor-epinephrine with increase sympathetic output with rise in free fatty acids levels. Direct alcohol toxicity to heart muscle causes a well known entity called alcoholic cardiomyopathy with weakening of the heart muscle resulting in Congestive Heart Failure in addition to atrial and ventricular arrhythmia being more prominent in those with known coronary artery disease with prior heart attack or weakened heart muscle. In these instances, there is an increased risk of sudden cardiac death.

Most common presenting symptom is having palpitations, chest discomfort, passing out, or become short of breath acutely with rapid rate of atrial fibrillation the heart muscle will not have a chance during its resting phase of the cardiac cycle known as ( diastole)and be supplied with oxygen. More deleterious effects observed in elderly folks as they become more dependent on the atrial kick for filling the heart, thus will loose up to 40% of effective pumping function forcing the patient into Congestive Heart Failure and Angina attacks and if untreated atrial fibrillation and will lead to strokes especially those with structural heart disease.

If we add over eating, high salt intake rising blood pressure, smoking weeds, elevation of uric acid causing acute gout attacks with pain adding to excessive alcohol intake ( known as Binges) will increase the chances of having Cardiac arrhythmias even with those without structural heart disease observed in young otherwise healthy individuals.

So, the message here from a caring heart doctor who has seen and managed so many with Holiday Heart Syndrome cases, it is preventable by monitoring your alcohol intake , avoid over eating, limit salt intake, caffeine intake, avoid dehydration and Holiday stress.

Follow this humble advice will save you from spending your Holiday Season in an Emergency Room or lying in a Hospital bed. Have a safe and Happy Holiday season!

Transitioning-from-Obamacare-1200x800.jpg
15/Dec/2018

In less than a month since the Election results came out on November 8th, there is a fast transition taking place with the new administration including healthcare. Moving forward from Obamacare to Trump Care would include important changes which will attempt to highlight some of them. First, keeping two important elements of Obama care ( No denial of coverage for Pre-existing conditions) and dependents up to 26 years of age can still be covered under parents.

It is unlikely to replace Obama care but repealing it would be a more likely scenario. The incoming Health and Human Services Secretary is a Physician ( Orthopedic Surgeon) Dr Tom Price who is also a legislator would be more likely to be more supporting to healthcare in general and doctors in particular. It is hoped that these changes will be gradual to minimize the potential adverse outcome.

So, it is expected using free market principles to increase access to healthcare that is more affordable with better outcome through delivery of quality healthcare. This will mean eliminating the Individual Mandate that individuals must buy insurance and allow this to be done across State lines without restrictions. In addition, allow individuals to get Tax credits from their Health payments up to $2500.00 per one year instead of giving government subsidy for healthcare based on income scale . Encouraging young people to enroll in Health Saving Accounts(HSA) which can be used by any member of the family.

Encourage competition among Drug Providers to offer safe, cheaper and reliable products( generic medication for example) in a free Healthcare market to bring prices down for the benefit of the consumer. Fighting Fraud, Waste and Abuse in Healthcare is also very important to help fund and sustain Medicare and Medicaid along with a shift to pay for value and quality healthcare rather than just volume linking outcome and other metrics to reimbursement of services.

No one disagrees that there is a lot of anxiety and worries about the fate of coverage of 20 million Americans currently enrolled in Obama care and has possibly fueled a surge in enrollment for 2017 as repealing Obama care may cause millions to loose coverage if legislation passes!

The most effective solution here is having more choices in a Free Healthcare Market where the consumer shops around for the best deal based on quality healthcare, customer satisfaction and price transparency that makes their healthcare truly Affordable.

With the added Incentive of giving Tax Credit for patients enrolled in Membership Medicine,or Concierge Medicine to save money on healthcare and get a tax break, will make Concierge Medicine and Direct Contacting for Healthcare for Individuals and Employers very attractive option especially with many Insurers pulling out of Obamacare. It is going to an attractive choice rather than forced Healthcare linked to Tax Penalties for not enrolling.

This will mean good news for Independent Physicians to fill the gap in healthcare delivery and thrive as all other options will not be compatible with their long term economic sustainability!
